Moms on Maintenance: Best Practices for Medication for Addiction Treatment (MAT) in Pregnancy

Overview:

Pregnancy is a time when women with substance use disorders (SUDs) are often highly motivated for recovery so their growing fetus can be born a healthy baby.  However, achieving meaningful recovery in pregnancy can be tricky.  Visibly pregnant women are subject to even more stigma than the general population of people with SUDs.  This stigma is present when they are seen smoking, drinking alcohol, or using drugs as well as when they are accepting treatment!  In addition, many women have had negative experiences with child protective services which leads them to fear the impact of maintenance treatment sometimes more than the consequences of ongoing substance use.  This training will focus on the evidence related to treating nicotine, alcohol, and opioid use disorders in pregnancy.  There will also be discussion of how child protective services legislation and personal autonomy play a role in a woman’s decision to accept gold standard MAT during pregnancy.

Learning objectives:

  1. Describe evidence-based treatment options for NUD, AUD, and OUD during pregnancy.
  2. Explain why maintenance medication for OUD during pregnancy is preferable to medically-supervised withdrawal.
  3. Provide a framework for collaboration with child protective services that supports moms choosing evidence-based care for their SUDs.
  4. Explore the concept of personal autonomy and harm reduction when gold-standard treatment is not chosen.
